Management & Leadership Essay

The way the world now developed through the industrial revolution from the twentieth century has a great background with many driving factor involved. The scheme that is cost-effective and has a good impact on social welfare of society, and those living in the society, is considerably dependent on the effective and resourceful piece of organizations of all kinds, which depends on satisfactory or excellent management and leadership potential, and which can be learnt and extended, would be acknowledged by many. This is the reason different government and government agencies nationally or internationally, organization of all kinds, investing in their own development, behaves as the basis of this belief that they make considerable investment in management and leadership training, development and education. There was a report in November 2006, Yahoo manager Brad Garlinghouse issued memo that directly challenge the senior management. He accused the Yahoo’s leadership lacking strategic direction. Their competitor Google had beaten Yahoo in terms of revenue, advertisement and the share price fallen nearly a third since the start of the year. He suggested to have a change in their strategy. Off course it is very difficult to change the strategy for any organization but to survive in the competent world a change should be made when needed. It was Mr. Garlinghouse great management and leadership skill that he diagnosed the problem and gives his organization a direction to move. Apart from what he observed and analyzed, the main part is that he made a decision to apply the change. He might have adopted different style for management as well as leadership, according to the organizational structure of Yahoo, and would be aware of what impact would be coming in applying the decisions. It would have been Autocratic or Paternalistic or it might be the Democratic one but what actually he identified was the lack in the management and leadership directions. Waka International is a multinational organization making FMCG (Fast Moving Consumer Goods) all over the world and for the last few years making its way up but not with a pace with which it has to moving over with good management and leader but still the HR management asked junior researcher to compile a report on how the managers can lead their team using different styles of management and leadership and how these styles going to impact on the strategic decision, and this is what good organization do to look what changes they need or things that can be adopted like Yahoo manager Brad Galinghouse did. There is a good saying by someone that there is one thing constant and it is â€Å"change† and researchers have made efforts to put forward some suggestion what style we should adopt in different work situation within a team in Waka International to meet the challenges on different stages. Management is been described as the process of setting objectives and making most efficient use of financial, human and physical resources to achieve these objectives, in which main task include planning, organizing, controlling and coordinating. The resources may include the organizational assets which may be people, machinery, raw materials, information, skills and financial capital to measure the organizational performance i.e. how efficiently and effectively management use these resources to satisfy customers and to briefly explore or examine management and in this report will be looking on the management theories of two recent writers. First researcher tried to sketch the attempt of Peter Drucker for his work to describe and comment upon what managers do and he identifies three tasks: Satisfying the goals or mission of the organization. Enabling the worker to achieve and focus on productivity. Managing social responsibilities. He suggested in simplified manner that managers are required to set objectives; organize; motivate and communicate; measure and develop people. Secondly to mention the influential work on management of Henry Mintzberg in which he details his observations of what managers actually did. He examine that managers were not found to spend most of their time planning, organizing, coordinating, commanding and controlling and suggested ten roles managers often attempt logically in the pursuit of their objectives and divided these roles in three stages as building relationship with the employees, yielding the quality information and finally the allocation of decision making, objective and resources to the employees. On the other hand leadership can be suggested as, the part of management to drive the organization to achieve its goals by utilizing potential influence with group efforts. Leadership in the organizational role involves establishing and sharing a vision, providing information, knowledge and methods to realize that vision so others can follow willingly. If we link management and leadership we can say that leadership is an essential part of effective management, which emphasis upon the building an environment in which everyone has the opportunity to develop and excel skills. This link between management and leadership shows that managers must have traits of a leader. While learning about leadership researcher describes two types of leadership which are Direct Leadership & Indirect Leadership. Direct leadership is something about imposing orders to your followers and maybe enforcing them to accept you as a leader, where followers used to see the leader regularly face-to-face. Or it can be said that the leaders who are able to get to know those they are working with and influence them on an interpersonal basis are direct leaders. While those having thousands of followers without possessing the role of leadership is the indirect leadership. It may also be believed that the indirect leadership has to do with influencing others and making them your followers indirectly by inspiring the followers through your character, charisma etc. Leadership is not about ruling people like subjects and taking it for granted; it’s about presenting yourself as someone who people think should be followed for guidance. It’s about winning the minds and hearts of people. If we like to differentiate between management and leadership we can come up with the idea that Managers perform roles in organizations and hold a specific, formal title and fulfill a role well Leaders aim to influence and guide others into pursuing particular objectives or ideas of the future and to motivate them into wanting to follow. In Waka International the management has the best quality leadership hence the reason, management asked their junior staff for such an important part to look on it, since the organization is willing to move up and each management is leading its team in a good way, with a good pace as well, in a better management and leadership style. Leadership style is the way in which the functions of leadership are carries out, the way in which the manager typically behaves towards members of the group. Different leaders have different behavior which might produce different effects, so researchers began studies on successful leaders and began to develop different styles of leadership. Also how to become a good leader one should be aware of his leadership style. There are many theories on leadership styles; these are summarized into three dimensional theories which describe different leadership styles: The concept in this style is of leader and followers, where leaders have got all the high power, authority and control; they make the decisions in a one way communication and direct their followers with minimal participation of followers. All the planning and thinking is done by the leader and the team’s role is to follow the orders and motivation to the followers only comes from the leader’s status, charisma, expertise and power to reward and punish. An example of authoritarian styles is John F. Kennedy and one of the many things that John F. Kennedy is remembered for is his vision concerning the United States’ space program. On September 12, 1962, while speaking at Rice University, President Kennedy said: â€Å"We choose to go to the moon in this decade and do the other things, not because they are easy, but because they are hard, because that goal will serve to organize and measure the best of our energies and skills, because that challenge is one that we are willing to accept, one we are unwilling to postpone, and one which we intend to win†¦Ã¢â‚¬  He then went on to talk about â€Å"metal alloys that had not yet been invented† that are â€Å"capable of standing heat and stresses several times more than have ever been experienced.† He had a vision of sending a man to the moon and back safely. He even explained how it was going to be accomplished. John F. Kennedy was exhibiting an authoritative leadership style that mobilized the resources of an entire nation towards this single goal. When we are talking about democratic leadership, it does not refer to a leader who holds political office or background, such as senator or parliamentarian. It talks about a leadership style that exhibits democratic characteristics. This idea of leadership involves running a business on the basis on the majority of decisions i.e. leader encourage the involvement of employees to take part in decision. Many of the great organizational leaders did use this style, Bill Gates is well known for his democratic management style at Microsoft. He actively involved employees in key management and strategic decisions. He was always interacted with the employees of Microsoft and customer and devoted his time for them by staying in contact via e-mail. In the democratic leadership style, we see a balance in the decision-making process. Employees or followers are allowed to participate in the decisions – their opinion counts just as much as the leader’s. Now this might sound like the optimal way to lead but every ways got some pros and cons in it. In this type of leadership managers observes that employees are working well on their own and consciously make a decision to pass to the member to allow them freedom of action and not to interfere and is always available to help in need. The leader may also set some example but nobody has to follow it if they want to. They leader don’t try to motivate their subordinate or it can be said that motivation only comes from the fact that the individuals are free to do what they likes and no one has to waste time explaining their actions. This leadership style may work with individual people like academics and artists who need to be free on work in their own way, and don’t like teamwork, they would like to like what they want, and not ask for guidance or permission. Researchers suggested that a specific leadership style cannot be constraint to a particular situation rather any styles can be adopted at any situations. Researchers also suggested that the autocratic style may be suitable when large numbers of un-skilled employees are involved and when quick decisions are a matter of life and death and have to be made quickly. Motivation is just a name which comes out from the personality of the leader that is the reason followers struggle to please or satisfy the leaders. So this style may be adopted when there is a need of urgency in Waka International and when a particular objective is achieved a different style can be implemented, and this is what management is all about, â€Å"the change†. Since in the democratic style, employees or followers have an equal say in the decision-making process, they are more dedicated to the desired outcome. The collaborative environment created by this style often results in more comprehensive solutions to problems. If Waka International is looking for better and long term result democratic style would be a better choice as most of the renowned company focus on the democratic style so it would be valuable for long term objectives and goals. Laissez-faire type of leader may inspire or motivate people or plan ideas but do not make any efforts to ensure that the team follows it. This kind of leadership can only work with academics or artists, who don’t like team work and emphasis to work freely. So many researchers don’t suggest adopting this style of leadership and it will not be driving the organization to its objective quickly. But this style is has good result if professional and experienced followers are in the workplace. From the report it can be concluded that management and leadership are two different aspects, but on a broad vision if leadership has not become the part of the management, this may lead to disastrous result. Coping up the resources and not leading in a good direction without an effective leadership may show short-term good result but it would not demonstrate a good impact on the organizational objective or goals and without the right leadership, organizational strategies will remain as ink on paper. Getting the right leadership to implement business strategies takes careful planning and dedicated effort, and often substantial investment. Like the sports coach whose mediocre team never wins a championship, the CEO can’t hope to achieve bold new strategies without giving thought to his leadership team and leadership culture. Too many organizations have become unworried with their existing leadership programs, instead of making certain that they are linking their leadership strategies and development investments with their business strategies and taking full advantage of the learning opportunities that accompany strategic change. With learning and research it can also be concluded that the planning methods are useful in meeting the challenges of any situation and developing leadership skills. Leadership traits can be learnt and developed to meet the strategic needs. This report has the relevance for handling complex situation of the present competitive business world by utilizing leadership skills in making strategic decisions to achieve the stipulated goal. The different styles mentioned earlier can be used depending upon the organizational needs and objectives, to achieve the goals.

A Boy is A Man in Miniature Essay

â€Å"A boy is a man in miniature, although he may sometimes exhibit notable virtue†¦ he is also schemer, self-seeker, traitor, Judas, crook, and villain – in short, a man.† (Davies 9). The theme of being twice born is prevalent through the novel Fifth Business and is strongly demonstrated by the characters, Dunny, Percy and Paul. All three change their names, deny their past and become what their parents could never have imagined. Consequently, at the end of the novel, the characters come full circle, revealing the same boyhood traits they portrayed years ago and are ‘thrice born’. To begin, Dunstable Ramsay began in Deptford, and as a result of his relationship with his parents, specifically his mother, Dunny needed to reinvent himself. His first step was to remove himself from Deptford and join the army and it was during the war that Dunstable became born again. In the hospital, Diana decided that Dunstable â€Å"†¦sounds like a cart rumbling over cobblestones†¦Ã¢â‚¬  (Davies 85) so she gave him the name Dunstan. During this time, Dunny also discovers that his parents had passed away during the war, but Dunny says, â€Å"I felt the loss so little† (Davies 74). As a result, all his strong ties to Deptford had been cut. As Paul says at the end of the novel, â€Å"I can’t imagine your parents foreseeing that you would become a theorizer of myth and legend†¦ Hard people – especially your mother† (Davies 253) which is true; Mr. and Mrs. Ramsay would never have thought that their son would have become a writer of saints, with a new name, completely different life from his roots in Deptford. Moreover, Liesl says to Dunny, â€Å"One always knows the twice born† (Davies 217) and Dunny is clearly in that group. Similarly, consider Percy Boyd Staunton. Percy never liked the small town and a turning point for him is when he is caught â€Å"in the act† with Mable Heighington causing his father to decide to send him to an all boys school, removing Percy from Deptford. His father was a local doctor and entrepreneur, but Percy went far beyond him. At Leola and Percy’s wedding, for example, Dunny states, â€Å"Boy had far surpassed his father in ambition and scope. All he needed was time.† (Davies 111). â€Å"Boy† was Percy’s new name after the war, â€Å"because he summed up in himself so much of the glory of youth in the postwar period.† (Davies 103). As time passed, Boy became  increasingly separated from Deptford. His final ties with his past were severed by the death of his father, as Boy had no reason to look back anymore, only forward, only up. His father would never have imagined what Boy would become, for as Dunny states, â€Å"Where his looks and style came from I never knew; certainly not from old Doc Staunton†¦ or from his mother.† (Davies 103). Boy changed his religion, much to the distaste of his family, created an empire from sugar and tried his luck in politics. It is through these changes that Boy becomes twice born. The final character is Paul Dempster, who denied his past very quickly by running away with the circus during his childhood. Paul states, â€Å"I was too young for the kind of guilt my father wanted me to feel†¦ I couldn’t stand it† (Davies 251) so he felt he needed to leave and the circus was his first opportunity. He changed his name to Magnus Eisengrim as part of his magic show. Paul had never been back to Deptford since he left and when Dunny asked if he would like him to tell his mother he was alive, his response was, â€Å"She is a part of a past that cannot be recovered or changed by anything that I can do now.† (Davies 139) showing that Paul had completely disassociated himself from his past. He was the son of a Baptist minister, he should have grown up to be an example to everyone else in the town of how a person should be, but he became a magician instead. Paul was â€Å"†¦a poetic magician who took himself seriously.† (Davies 192) with an act like no other. It is clear that as soon as Paul ran away with the circus, he became twice born, but throughout his life, he was followed by the blame for his mothers’ madness. Finally, by the end of the novel, the characters have come full circle and in a meeting in Dunnys’ office, they discuss the matter of the snowball. Dunny becomes thrice born when he tells the truth about the snowball, relieving a burden that has governed him throughout his life. Dunny has always felt the same boyhood guilt and had finally come to terms with the incident. Paul was also thrice born as when he hears the story, he comes to realize that the blame that was on him as a boy for his mothers’ madness was not truly his. This brings into question, Boy. Did he in fact become thrice born? Any reader would like to believe so, and believe that he killed himself out of  guilt, but who can say for sure? He did say, â€Å"I wish I could get into a car and drive away from the whole damn thing† (Davies 232) But even as he left, Boy denied remembering the incident of the snowball and said that he did not feel any guilt. Furthermore as Paul was leaving, he said, â€Å"I have everything I need† (Davies 255), was he referring to the story or was he referring to the stone that was in the snowball?In conclusion, the concept of being twice born is the vessel for development of the characters in Fifth Business and is a reoccurring theme throughout the novel. Before the meeting Dunny says, â€Å"The cloaks we had wrapped around our essential selves were wearing thin† (Davies 233), suggesting that the concept of ‘twice born’ is not a permanent change but a means to come full circle and revisit their boyhood. During the meeting in Dunnys’ office, Percy says that boys are brutes because they do not know any better, but they grow up to be men, and Dunny replies, â€Å"Men who retain something of the brutish boy † (Davies 254). Boys truly are men in miniature, they can change appearance and their name, but they will always come full circle and realize those brutish boyhoo d traits in the end. Bibliography Davies, Robertson. Kafka’s Metamorphosis in Context to His Era Essay

  Ã‚  Ã‚  Ã‚   One of the major German writers was a Jewish, middle class resident of Prague, a man named Franz Kafka, who wrote disturbing, surreal tales. Writing in both short story and novel form, his work was published posthumously by a friend, Max Brod, who ignored his requests to burn his writings upon his death. Because his friend disobeyed his last request, Kafka’s work has become iconic in western literature, even producing its own connotations.   The term â€Å"Kafkaesque† has come to mean mundane yet absurd and surreal circumstances of the kind commonly found in Kafka’s works (â€Å"Kafka†,1).   Ã‚  Ã‚   One of the most widely read and famous of these works concerns a man who wakes up one day and discovers he is an insect. Literally. Known as Die Verwandlung or The Metamorphosis, Kafka wrote this story quickly, completing it between November and December 1912.   Ã‚  Ã‚  Ã‚   Because of its bizarre subject   matter, his tale has been subjected to a wide variety of interpretations. Although critics vary widely in those interpretations, the basic story involves a man who awakens in different form: he is now an insect; a â€Å"giant monstrous vermin;† yet all he wants to do is get to work. He has provided for his family and feels the pressure of helping them even now. However, in this new context , he cannot speak with his family members. Judging only by appearances,   his relatives becomes repulsed by him, calling him a burden. Each time he enters to try to be in their midst, they act mean; his father even goes so far as to throw an apple, which subsequently gets infected after it embeds in his back. Although Gregor becomes a veritable prisoner of his dirty, grimy room, his family does provide food and other nourishment-for a time. But they so abhor his appearance and treat him so despicably, that his sister finally declares that   â€Å" that thing must go.† His mother doesn’t even offer a word of protest. Because of his outsider status with his family, Gregor returns to his room one last time; desirous of relieving them of their burden. He lies down. And dies.   Ã‚  Ã‚  Ã‚   Both the structure and the setting of the story resemble that of a drama. The structure builds dramatically, with a series of three crises, leading to a denouement. Each section of the story has a defined area where the story takes place; a limited space as in plays.   With the exception of Gregor, the other characters are one dimensional.   Ã‚  Ã‚  Ã‚   Thus, Kafka works out of the traditional Aristotelian framework of three acts consisting of a beginning, middle, and end. Yet his style is ordinary. Has he been overrated? His plot is limited in scope, a series of episodes in the life of a character, rather than a full development. The characters are also limited. So what exactly did cause this Kafkan phenomenon?   Kafka dealt with the subject of contradiction and the absurd— with a sense of impotence against the absurd conditions and banalities of the world. Although not attracted to any â€Å"isms’ of thought philosophically, politically, artistically, or religiously, he simply expressed his own soul (Artile, 1).   Ã‚  Ã‚  Ã‚   Despite his lack of referencing, the wider world nevertheless laid claim to him.   Ã‚  Ã‚  Ã‚   The Jews saw him as their own visionary. They were convinced he foresaw the arrival of the Holocaust. Yet Kafka was not a religious Jew, going to synagogue only four times yearly with his father and having a bar mitzvah at age 13. Too absorbed in his personal frustrations to pay much attention to political developments, Kafka could not help becoming cognizant of the increasing xenophobia and anti-Semitism of those around him. He thought that Palestine was a good solution and often talked of moving there to operate a cafà © with his girlfriend Dora. In the midst of the anti-Semitic riots of 1920 Berlin, he said that â€Å"the best course is to leave a place where one is hated† (Strickland, 2). Indeed, his own three sisters all died in concentration camps, a   fate that might also have awaited Kafka had he lived rather than dying of TB in 1924.   Ã‚  Ã‚   Although only a secular Jew, Kafka was nevertheless attracted to Yiddish theatre. The Metamorphosis has many parallels to a classic work of Yiddish theater called The Savage written by Gordin. The son Lemekh in this tale is â€Å"defective† like Gregor Samsa.   Outcasts who horrify, both characters are animal like creatures in decline. The central metaphor of The Metamorphosis corresponds to  Ã‚   Lemekh’s position in his own family. As the housekeeper states, ‘they kill him if he comes in here, so he lies in his own room, days on end, with his eyes open, and stares, like an animal, waiting to be sacrificed’ (Beck, 54).   Ã‚  Ã‚   Beck continues to state that the Oedipal conflict and the larger theme of incest is present in both works because the sons’ love for their mothers and sisters become confused with sexual desire. They become dizzy when they see their parents embrace. When Zelde touches Lemekh, he gets hot. Similarly, Gregor wants to save the picture of the lady in furs, crawling up the glass which soothed his hot body. Crawling shows his acceptance of his animal state- hiding when others enter, fainting- which intensifies the action and shows strong emotion. Lemekh in his iron jacket and Gregor in his armor plated hard back are both imprisoned, and spiritually limited. Gordin’s play warns of the beast in every man hiding beneath his human faà §ade. Kafka’s work also seems to be pointing to the vermin which every man inherently embodies (Beck, 56).   Ã‚  Ã‚   Other groups besides the Jews also embraced Kafka. Psychoanalytic Freudianism and   Existentialism saw reflections of their philosophies in his works. The Freudians saw every range from dreamlike qualities and Oedipal conflicts to symbolic odds and ids. Kafka’s feelings   for his own father reads like a transparent Oedipal story. Many critics were of the opinion that never before had Freud ruled so supremely over a story as he did The Metamophosis (Eggenschwiler, 72).   Ã‚  Ã‚   Existentialism took Kafka to be one of their own .Because he created characters who struggle with hopelessness and absurdity, many in the movement saw him as an icon, while others in the group were disillusioned with the western status quo of the 50s and the 60s. They distorted Kafka by exploiting the heavy atmosphere of his stories, using them as the basis for the need of a more liberal society with less state intervention and more truth for the individual.The existentialists abused truth by portraying a psychotic Kafka, victim of their same angst. The humor and mischief that was so dear to the surrealists that he loved is lost with that existentialist label ( Artile, 7).    One of the most obvious themes of The Metamorphosis concerns society’s treatment of those who are different and   the loneliness of being cut off; the desperate and unrealistic hope that isolation brings (â€Å"Kafka,†3).   Ã‚  Ã‚   In his pain and rejection Gregor Samsa was far from being everyman. And most readers will not be prepared to accept him as a universal symbol. Nevertheless, it is hard to avoid the condition in The Metamorphosis that Kafka was demonstrating; at least at that time; his own despairing, tragicomic vision of the human condition ( Beck, 57).   Ã‚   Kafka’s value will always lie in the inexplicable that it contains. Final understanding will probably remain an   impossibility. The various mid-century groups that took him as their hero never saw the complete picture of his artistic merits or original thought. Although many of his stories are inscrutable and baffling, Kafka himself looked upon his writing and the creativity he produced as a means of redemption (Artile, 7).   Ã‚   Thus his work transcends all the various interpretations that have been forced upon it and stands on its own merits, remaining an important part of the Western canon; work that is timeless. Changing Of Life - Essay Example In these regards, there were a lot of daily tasks that I took for granted. For instance, simple things like fixing my car I never took care of, because I had always relied on my brother whenever there was a problem that was needed. For instance, I recall one incident where we were driving to visit a family friend. During the trip, one of the tires went out on our car. My brother immediately got out of the vehicle, opened the trunk, and began to change the tire. After my brother's accident, he was not able to accomplish tasks such of this. A few months after his accident we were driving to visit another individual and a tire went out on the vehicle. For an instant, I looked to my brother expecting him to jump out of the car and change the tire but instead came to realize that this time I would have to learn how. We rolled the window down and my brother explained to me the exact steps I needed to take in order to change the tire. This has I learned the process and would be able to acco mplish it in future situations. While this situation with changing tires is one incident, it reflects the large change that took place in my life after my brother's accident. Slowly, I began to learn a lot of skills that I had never had to learn before. My life had truly changed. From this point on I would no longer be able to simply watch television or engage in random past-time activities. My new lifestyle involved becoming more active in day-to-day activities. I learned to become more engaged in daily activities.

The Human Rights Act 1998 is broken down into 5 key principles which are; Fairness, Respect, autonomy, dignity and equality. Equality would influence working principles because the children would be treated fairly by having their individual needs met to ensure the same outcome is achieved as the other children. Settings can maintain fairness and respect by having the practitioners in the setting following the same policies and procedures. This would also avoid conflict between staff and would not confuse the children with knowing their expectations. Another way settings can have respect in the setting could be by following parent’s preferences such as their food preferences for their child. My current placement follows parent’s preferences by having meals that are suitable for children without specific food preferences and set meals to meet cultural and health needs. Settings can promote autonomy by encouraging children to build on their life and social skills. This can be done by the setting providing games such as a simple board game where the children have to choose their game piece and where they have to turn take. This would also build on autonomy because it would teach children how to share and how to treat other children or people with a positive attitude. For an example if a child is an only child, they may not know how to share with other children, so playing a game that engages their interest would gradually teach the child how to build on his turn taking skills and how to interact positively with other children. In the setting, dignity can be maintained by removing situations where a child may feel embarrassed or ashamed. If a child wets their selves, the practitioner in the setting should take the child into the bathroom and change them. The practitioners shouldnt draw the other children’s attention to the child who has had an accident. The child would need some comfort and reassurance as this situation is almost always v ery embarrassing and distressing. The Childcare Act 2006 focuses on adult to child ratios, qualification levels of staff and types of drinks and snacks available for the children and follows the early year’s foundation stage framework. Settings can follow the Childcare Act 2006 focuses by  providing fruit, milk and water at snack time. This can also be done by having playtime outside so that the children would get vitamin D and fresh air. Also the settings would provide meals with nutrition and are aware of dietary and cultural needs. The children act 1989 and 2004 aims to protect children by working in partnership with parents and by the every child matters which have five main principles: Be healthy, Stay Safe, Enjoy and achieve through learning, achieve economic well-being and make a positive contribution to society. The Enjoy and achieve through learning will influence practice because it allows practitioners to cater for each child’s needs whilst ensuring that the children are enjoying what they are doing or learning. The Equality act 2010 has been simplified from 9 separate acts into 1 act. The main is to end all forms of discrimination. The protected characteristic of disability will help to influence practice as this will help practitioners be able to differentiate activities and to re-create the same experience using other methods or activities. The health and safety at work legislation is about keeping yourself and others safe. This will influence practice because it will ensure the utmost safety for both the children and staff of the setting. This can be done by having regular risk assessments and by finding a solution to any hazards. For an example, if there is spilt water, a staff member should mop it up as soon as possible to avoid staff or children slipping and being injured. E3. Describe how policies and procedures will help safeguard children. One policy could be the safeguarding policy. The safeguarding procedure helps to protect children because the setting would have a safe recruitment method to ensure that all staff are suitable to work with children or are qualified to work with children legally. For an example, before I could start my placement and working with the children, I had to show my CRB check to my nurserys placement officer. CRB checks helps to safeguard children because it would prevent anyone with a serious criminal offence or someone who could be a danger to children from working with them there for protecting the children from being mistreated or harmed. Another part of the safeguarding procedure that would safeguard children would be to have a designated person who has a clear and defined role and responsibility in regards to child protection. Having a designated person would help safeguard children  because they would be the person the practitioners should go to if there are concerns about a childs safety or welfare concerns about the actions of a staff member. This would safeguard children as the designated person would be able to decide what actions need to be done in order to resume the childs welfare or to decide what the disciplinary action is for the staff that has had the allegation made towards them. To avoid this, all staff should have regular child protection training. A separate policy could be the behaviour management policy. This policy would help safeguard children as it helps children to think about their actions and the consequences they may cause. When a child has behaved in an unwanted behaviour, the disciplinary guidelines should be taken. The disciplinary guidelines are Give verbal warning, Move the child away from the area and time out if the first two stages don’t work. The above verbal behaviour management methods should always only be used instead of physical contact. However, physical contact should only be used if you need to restrain a child if they are causing harm to their selves, other children and people or are damaging the settings property. It would help safeguard children because it would reduce the risk of the children being physically hurt and would maintain a safe learning and play environment. It is important that the setting outlines the behaviour because management procedures because it will give consistency to the children as the practitioner within the setting will follow the same set of rules enabling children to know the rules and expectations clearly. E4. Describe how the policies and procedures promote fair, Just and inclusive strategies. A procedure that promotes fair, just an inclusive strategy is adapting resources to meet children’s needs. This can be done by having a range of equipment that helps children and their individual needs. This can be done by providing a range of different sized paint brushes and sponges for an example. This would allow children to develop their creative skills and interests whilst being able to develop their fine motor skills. A policy that promotes fair, just and inclusive strategy is the equal opportunities policy. This policy helps to include children because everyone in the setting has the right to be included in everything the setting does and every child has the right to education and developing their skills. Equal opportunities policy will also help children to feel valued and have their  progress and achievements celebrated. This can be done by displaying children’s artwork, creations and other outstanding work on a display board for all parents and staff to see. Another procedure that promotes fair, just and inclusive strategy is by showing awareness of different cultures and life styles. This can be done by having related activities or circle time celebrating different cultures, religions, life styles and things that challenge normal expectations. For an example; ‘all nurses are female’. This is inclusive practice because it involves every child’s cultural preferences and builds on children’s knowledge and understanding about the world we live in. Another policy that promotes fair, just and inclusive strategy is the Special Education Needs (SEN) policy. This policy allows every child to have the same care, attention and consideration as any other child in the setting, regardless of their disability or background. The policy states that every setting should have a SENCO worker and responsibility in adapting and adjusting the settings environment to help children have the same experience in the setting whilst having their individual needs met. This can be done by having braille books for children with a sight impairment or provide easy access in the building for wheel chair users. E5.Describe two strategies which can be used to empower children to develop independence and self-reliance One strategy that could empower children to become independent and self-reliant could be by having circle time regularly. Circle time should allow children to express their thoughts and feelings with the security of being listened to. Another strategy that would enable children to be independent and self-reliant would be to take part in food preparations such as choosing what fruit they’d like for snack time and cutting the fruit themselves. Give examples of how settings may prepare children for transfer or transitions â€Å"transitions into reception classes was at best abrupt and worst traumatic† (Scott,(2005),P21) One way settings can prepare children for transfer or transitions could be by the child’s key worker taking the child to open days or visits. Another way setting can prepare children for transitions could be by having learning partners or mentors who are older children from the setting where some of the children may be moving up to. This would help children to build relationships so that they feel confident in moving up to the setting because they’d know another child there. Final ways setting can help prepare children for transitions could be by having individual transition plans for each child. The setting would need to â€Å"make time to share previous information† (Scott,(2005),P21) to the setting that the child is transferring to because this would allow the teachers to cater and plan for the child’s needs and interests. Explain the key issues relating to the practice which supports children to prepare for transitions One key issue relating to supporting children through transitions when starting nursery could be to follow a settling in procedure. The Settling in procedure often involves havin g the parent and child having a visit before the child’s official starting date. Having a visit before the child’s starting date will help the child to become more familiar with the setting. In time the child should feel more comfortable with going to the setting. The settling procedure also involves working in partnership with the parents. If the child sees the parent interacting with the staff members, it is more likely that the child will feel secure in the setting. Another key issue relating to supporting children through transitions when starting primary school would be to create a time where the children are involved. For an example, an activity could be to have a show and tell circle time, where the child brings in something from the weekend or something to do with the set topic and talks about it. This would help a child to settle in a primary school because it would give the child a chance to be involved with the class and have their discussions listened to. A third key issue relating to supporting children through transitions when starting nursery would be to find out what the child’s favourite toys are and put them out for the child to play with when they come into nursery. Also, to make the child feel welcomed, the practitio ner should also provide a range of appropriate activities for the children and always give the child an opportunity for them to participate in the activities as much or as little as they like. An alternative key issue relating to supporting children through transitions  when starting primary school would be to work in partnership with parents. This can be done by having notice boards around the school which has features such as â€Å"most improved child†, â€Å"Little stars† (Where children are placed here for good work or for doing something kind etc) and put the child who is starting in the primary school on the notice board. This would make the child and parent feel proud and would give the child a sense of achievement which hopefully would make the child want to enjoy and participate in the school more. An alternative key issue relating to supporting children through transitions when starting nursery could be by providing opportunities for the child to express their feelings. This can be done by having regular one to ones with the child and their key worker, Having areas for the children to independently draw and write in addition to having â€Å"End of the day† circle time where the children have the freedom to say what they liked about the day or what they didn’t like. This will help the practitioners in the setting know what the children really engage in and which activities they don’t like to participate in. Also It would help the practitioners learn more about the child who is starting in the nursery as it will help the practitioners provide more activities that the child is interested in. Hopefully in time, this will help the child to enjoy nursery more. A final key issue relating to supporting children through transitions when starting primary school would be to help the child develop self-help skills. This can be done by encouraging the child to take responsibility for their selves, such as putting on their coats or helping to tidy up the classroom. This would help the child settle in nursery because this give the child a sense of accomplishment and would also build on their self-esteem with the thought of â€Å"I can do that† and being able to try new things. The theoretical perspective links in with strategies that empower children because it helps children become self-reliant. For an example, at my placement during snack time, the children have a choice of milk or water. The children either has to point to or say the drink theyd like to have. This encourages children to language and encourages them to start to take responsibly for their own wellbeing. This theory also influences practice because it helps the childcare practitioners to define children who has specific needs or children who needs extra support as they may not be developing in the developmental areas as they should. The good element to this theory is that children learn to build on the childs life skills such as becoming more self-reliant. The theory is also good because play is important as it lets children use imagination and responsibility of the childs learning. It also helps to build on the childs fine and gross motor skills. However, the bad points about this theory are that children develop little without an adult although staff shouldnt allow the children to be too attached to them as the children wouldn’t interact very well with children their age as well as giving children adequate help and support so that the child can learn to be self-reliant so that in the future they, they would be able to achieve more without much support from the adult. The theory can be improved by ensuring that the staff should try and make sure that the sessions within the setting are 50% adult led and 50% child led as this would help the children to form knowledge and understanding of the world because of accurate information being shared by the childcare practitioner and will allow the children to take an active role in choosing what theyd like to learn. For an example, a child may choose to play with toy animals. To a person, the child is just playing but the child could actually be learning about the different types and sub-species of animals in a basic and simple form. In addition to this, having a balanced adult and child led setting will help children to get an equal opportunity to interact with both adult and children. E7.Exaplain the cause and effects of discrimination on children B1.Discuss  how the causes and effects of discrimination may affect practice in the setting. The staff attitudes and values can affect the atmosphere because the childcare practitioners may unintentionally teach children that it is okay to discriminate people. For an example, if a member of staff said â€Å"all ginger children are violent and aggressive†, this could influence children because it may make the children tease or leave out other children who have ginger hair during play time. It also may affect the way the practitioners work and teach because the practitioners might interact with a group of children and leave out a child because of their views. This affects the children because it â€Å"children can quickly pick up the signs given out and they can sense if there is tension† in addition to making the child feel undervalued and not accepted which can make the child withdraw from activities and communicating with other adults and children or could trigger bad behaviour. Settings should show respect for the parents and families by respecting the parents or families’ choices and requests within reason as this would show inclusion. For an example, if a parent prefers their child to be vegetarian the setting should provide vegetarian food whereas if a parent asks for a child not to play with a toy/or do an activity, the staff should challenge it every child has the right to have an equal opportunity. This can affect the practice within the setting because the practitioners may have a stereotypical view about children such as girls should wear dresses and play in the home corner whereas boys should only play with construction activities. This affects the child because it would make the child feel like they are not unique as they may become fearful of doing new activities as they have been told off or been influenced to only do what the adults say. In my nursery they avoid being discriminative by providing food options suitable for a range of cultural preferences, the setting will make sure that the child will have a vegetarian meal. Discrimination can affect children because it can make them feel awkward if there aren’t many posters, books, toys or other resources that would help other children learn about diversity, different cultures and different lifestyles that may challenge the ‘norm’. Apart from a geographical area a community is a group or society, helping each other. In every society various types of communities may take place. Some categorizations are as follows:- Geographic communities: It ranges from the local neighborhood, suburb, village, town or city, region, nation or even the planet as a whole. These refer to communities of location. Communities of culture: It ranges from the local clique, sub-culture, ethnic group, religious, multicultural or pluralistic civilization, or the global community cultures of today. They may be included as communities of need or identity, such as disabled persons, or frail aged people. Not on the basis of above things, There are different things that bound people together to their different communities or that they may have in common with the other people living around them, which includes beliefs and values, traditions, common interest, language, territory, religion, culture and occupations. Community Mobilization: A strong community feeling one can identifies at the time of crisis and occasions like change for families, such as marriage and death. In a community people shared their we feeling with each other. As we know that proper management and utilization of resources is the best possible way for the development of the community. Therefore community mobilization is always prominent for the development and upliftment of any community. For mobilizing the community people make their plans and then do the things accordingly. They take charge, transforming, developing their community and their lives as well. Community mobilization is a process which allows people in the community to identify their needs and problems and bring them together for a social action and their community development. Communities can mobilize to work for changes that will fulfill the social, emotional, financial and physical needs of people. Community mobilization is an organized process through which people can aware regarding the present situation of their community also how to bring positive changes within the community by using various resources. Nutshell, community mobilization is one of the strategic method which helps the people to attain maximum community development. For community mobilization communication is one of the important tool to achieve it. Through proper and effective communication various methods of mobilizing the community can be implemented. Here we are discussing the various methods for community mobilization by given actors as per the fig 1.1. Mobilizing the community Political Parties Social Movements Individual groups Political Parties The use of social networking sites by political parties and other politically active groups has been increased in these days. All these political and other groups realized that by suing of social networking sites many people can be mobilized. The internet has become the strongest communication tool even at the global level. Even various politicians also used Twitter, Flicker, You Tube, and Facebook among people to communicate and mobilize people. As political parties using of social networking sites is one of the effective method to mobilize the people. In the present scenario, the use of social media is really important to mobilize the people and communities for collective action. We know that today millions of people are connected in one way or another to the internet. People are easily accessible on internet. It is another way of reaching large numbers of people to mobilize them for a group effort. Facebook, Mixit, Twitter can also be effective for the above. To generate awareness among people and to mobilize them in large number to make a community independent. Newspapers, radio stations and the TV have the authority to inform thousands more people about various campaigns for mobilizing any community. By writing a press release, a political party can highlight the facts, peoples demands, and what people want the media to talk about regarding the development and continuous empowerment of their community. Parties can also organize a press conference. So they can invite the reporters to come to a meeting where spokespeople from the community organization speak out publicly about the problem and the solution of the problems of community. Apart from above, Political rallies and the use of local languages are instrumental in the mobilization of groups for social action. Social Movements Social movements are a type of group action. They are large informal groupings of individuals or organizations which focus on specific political or social issues for their development. In other words, they carry out, resist or undo a social change in the society. Social movement campaigns play a critical role in mobilizing and maintaining the submerged networks of social movement communities. A social movement refers to collective effort organizing from the people for their development and to bring social change in the society. Community mobilization is often used by grassroots-based social movements, which includes revolutionary movements. Through Social movement for mobilizing the people in community we can apply various methods like large public gatherings such as mass meetings, marches, parades, processions and demonstrations. These gatherings usually are part of a protest action. These are the different ways by which a community can be mobilized and can raise voice for the demand of development of the community. In other words, community mobilization seeks to facilitate change within the community for its development. Modern Western social movements became possible through education (the wider dissemination of literature), and increased mobility of labor due to the industrialization and urbanization of 19th century societies. Social movements have been and continued to be closely connected with democratic political systems. Occasionally, social movements have been involved in democratizing nations, but more often they have developed after democratization. Modern movements often utilize technology and the internet to mobilize people globally. Adapting to communication trends is a common theme among successful movements. Research is beginning to explore how advocacy organizations linked to social movements use social media to facilitate civic engagement and collective action. Social movements are any broad social alliances of people who are connected through their shared interest in blocking or affecting social change. Although social movements do not have to be formally organized. Social movement is always an important tool to bring the change in society and as well as to mobilize the communities for their own development. By arranging people in mass meetings, parades, marches, discussions on various issues, movements for stressing the education of people, bringing the concept of democratization in light communities can mobilize through theses social movements. Social movements are always a part of society, and people may compare their options and make rational choices about which movements to follow. As long as social movements wish to be success, they must find resources (such as money, people, and plans) for how to meet their goals. Furthermore, for organizing successful social movements and mobilizing the people there should be strong and effective networking with other organizations especially the organization which are exist in the local community. There has to be proper networking and strong communication even on the grass roots organization which are working locally in the community such as Church groups, clubs, special interest organization, local schools, hospitals , recreational groups and other community groups can support the social movement and through effective and strong networking with the above organizations we can mobilize the communities. Physical concentration is also necessary for organizing successful social movement and for mobilizing people. We should bring people into closer proximities in cities, factories, local university campuses which help the social movement to get into the right direction for mobilizing people for their betterment in society. Also a co-optable communications network should be there to speed up the social movements for mobilizing the people and to spread awareness among the people. As we know that the greater the number and excessive participation of people builds a strong network which leads to mobilizing the greatest number of people. Also for maintaining the above and to get a fuller participation of people and to mobilize them we need to able ourselves to maintain the resources within and outside the community. We should also provide the equal opportunity to each and every unit of the community. We cannot let the youth and other units to go. We need to encourage every individual with n o matter of his / her own personal identity. We need to motivate people to participate actively in the social movement and consider the point of availability in our mind. As People with full time jobs, marriage and family responsibilities are less likely to participate in social movement activity. Autonomous individuals with few personal responsibilities such as college students and single professionals are much more likely participants. On the other hand, for mobilizing people without considering them as particular unit of society, we need to encourage them for their maximum participation. Through motivation and encouragement we can mobilize people and can make them ready for their community development. Individual groups It is always easy to start a fire and involve a number of people. Community members need to choose certain people whose role will be to keep the fire of community mobilization burning. In this guide these people will be called motivators. People in the community know each other very well. Take time to carefully choose honest people who are respected by others, and who can encourage others to work well together. They need to have some time and energy available, have a desire to bring change and be willing to work free of charge. Each community should select two or three motivators. The people chosen will need equipping and support in their role. Each local area has a number of different kinds of resources. People use these resources to keep alive and to cope with changing seasons, political change and cultural pressures. Helping people to understand and to value the different resources they have is very important. These are the main kinds of resources: _ Natural resources include land, trees, forests and water. _ Human resources include the skills, knowledge, understanding and labour of local people. _ Financial resources include money, access to credit and loans, credit unions and government support. _ Social resources include the culture, traditions, organizations, friends and extended family. _ Physical resources include buildings, tools, roads, water pumps and transport. _ Spiritual resources are the strength and encouragement that people gain from their faith. Local people already know more than any outsider about their community and the people living there. Many people assume they know everything about their local area, but there is always more to learn and understand before making new plans. Take plenty of time to help people in the community to tell their story together. One very effective way of doing this is to draw maps, either using clear ground with sticks, leaves and stones, or if available, large sheets of paper and pens. Encourage small groups to draw different maps to show: the natural and physical resources in the area (hills, forests, roads and rivers, for example) -where people live, noting important people and organisations -how the area looked 50 or 20 years ago (only for older people). So for mobilizing communities, individual groups can do the social mapping of the community so that the people can aware about the present problems in the community. Another helpful way to focus on key issues within the local area is to encourage people to prepare a role-play to express their concerns. As people discuss what subjects to use, they will often focus on important issues. However, they are also likely to share these in funny ways. Laughter has a way of taking the pressure out of a situation, helping people to discuss sensitive issues, sometimes for the first time. Again, encouraging people to work in small groups is another way to mobilize the communities. Having chosen their first priority for action, local people need to decide whether they have enough information to take action. For example, if education is identified as the priority, people may need more information about the problems and whether they are at primary or secondary level. The problem may be poor attendance and, if so, there is a need to find out why. Considering electing teams of local people to gather relevant information is another way to mobilize the communities. Choose people who can be trusted and who know their community well. Before sending teams out to gather information, take plenty of time to decide exactly what kind of information is needed. Apart from this, conducting role plays- street plays in the community is always a good strategy to mobilize the maximum number of people and to preparing them for a social action. By Arranging a community meeting to share all the information gathered is showing information clearly to the people. A large number of people can mobilize for the collective action after getting all the facts and problems of the community. Also, motivation plays an important role in this activity. As keep motivating the people in right direction is always worthy and will be fruitful for attaining maximum development of the community. Initial contact with the community is another way to mobilize people. Understanding community practices and traditions prior to establishing contact can help identify the appropriate approach for engaging with different groups and members of the community. It is important to focus on learning from the community, especially during the initial contact.  Taking every opportunity to meet informally with diverse members of the community (at the health post, during registration, at distribution points, in the queue for water). Also utilizing community leaders also increases the chances of reaching a cross-section of the population and not just a narrow representation. It is essential that all members of the community receive word about mobilization and how they can be involved.  Outreaching to those who are isolated, vulnerable or considered. If they cannot personally attend a meeting, it is important that they be represented in some way. So as per the above discussion, motivating peop le, arranging meetings, initial contacting with the communities, utilizing the community leaders, conducting role-plays, street plays, community outreaching are different ways to mobilize the maximum people of community by individual groups. Also, identifying an existing committee or a community-based organization which can give individual groups access to the community and facilitate distribution of  messages. Meeting the host community and the authorities is important to reach out the goals for community development and mobilization.   Messages might only reach certain groups, such as community leaders, and not all members of the community.  Developing outreach strategies with the leaders and others to ensure that everyone is informed, including women, girls and boys, minority groups, and people with mental and physical challenges. Making sure that information is delivered in a language everyone can understand, is culturally sensitive and is correctly perceived and understood.  As always communication should be effective and strong for the mobilization. Arranging meetings at mutually convenient times. As far the mobilization concern, for development of any community individual groups should make positive rappor t building among the community with key people- like facilitator, leaders, service providers etc. and whole community as well. A good rapport building is effective strategy to mobilizing the people.  Ã‚  Ã‚  Transparency, respect and consistency are essential for building trust, confidence and collaboration between organizations and partners, including members of the community.  Also individual groups have to make ensure that after the first contact immediate follow-up action is taken or not.   Orienting the Community is another way to mobilize the people. The first step in mobilizing the community is to orient them to the process.  This can be done in a number of ways, including written communication, television, radio, or an organized meeting.  Meetings are more personal and conducive to building relationships.   They also facilitate two-way communication where questions can be answered efficiently. As groups can motivate community people for their maximum participation in every discussion as they should feel that they are the key part of it.   Also to start these meetings as soon as possible, so that trust and a positive working relationship can be built between parties. Having the meeting sponsored or hosted by a respected individual or group within the community can add credibility to its agenda.  This may happen through a church, school, tribe, or other local group.   Utilizing community leaders also increases the chances of reaching a cross-section of the population and not just a narrow representation. It is essential that all members of the community receive word about mobilization and how they can be involved.  Outreach is prominent to those who are isolated, vulnerable or considered marginal is critical. If they cannot personally attend a meeting, it is important that they be represented in some way. Individual groups should take care of these things before conducting any activity as part of the community or within the community. An only arranging meeting is not as sufficient. The goals of the meeting must be carefully considered and reflected in the agenda.  Community leaders can provide assistance presenting the information in a culturally appropriate manner. Some of the community leaders may be chosen to convey topics with which they are familiar.  During this meeting, it is important to invoke the input of the community. Identify their needs and begin to priorities how those needs will be met.   Individual groups sho uld know that this meeting will be an opportunity to develop an awareness of what the contributing organizations can provide, and also an opportunity to learn about the strengths and resources of the community. It will be important to define mutual goals and develop a plan as to how to reach these goals.  This will include organizing individuals to work together and coordinate services.   Proper organization of every resources and management is necessary to mobilize the people. Also individual groups should develop ongoing ways to communicate for mobilizing the people. Once goals have been defined and a plan has been developed, it will be important to identify ongoing ways to communicate.  Rather than having large meetings, cluster meetings of project staff, community leaders and community members working on similar tasks are more manageable and efficient.  The groups should begin team building with all team members (international  and national staff, community leaders and community members). It is vital that marginalized and vulnerable people are included in these teams.  Intervention with the community may be required for them to allow marginalized or isolated members of their community to participate in a team.   Working in small teams that include affected people as well as outside helpers.  Team leaders should meet daily for sharing of information, planning and coordination. Not all teams will be required for all emergency response projects. Some people may be a member of more than one team. Conclusion: It is to be concluded that for continuous community development mass awareness, people mobilization is very important. In our society there are different resources which can perform to mobilize the community. But one should only need to generate these resources within the community or outside the community. Maximum resource utilization and mobilization of people are key things for developing any community. Community development emphasizes participation, initiative and self-help by local communities but should be sponsored by national governments as part of a national plan.

Also patient is not able to perform daily activities due to shortness of breath and cough with sputum production. These symptoms are quite common in carcinoma of bronchus, however difficulty in breathing, cough and sputum production are also main clinical symptom of COPD (Pauwels and Rabe, 2004). Moreover, history of the client indicates that plentiful production of sputum for more than three months for two years which shows t he presence of chronic bronchitis (GOLD, 2008). Patient is also not able to sleep during night. Awakening during night which occurs by shortness of breath reveal presence of congestive heart failure or asthma (Price, 2010). Complaints of patient we loss of weight, fatigue, chest tightness due to severe episodes of cough, disturbance in sleep. Dietary history shows that patient is non vegetarian, family history does not provide any significant data. Client having age more than 40 and have sign and symptom such as formation of sputum, difficulty in breathing, history of cigarette smoking and any inhalation of noxious fumes and chances of occurrence of COPD is more at the age above 40 ,therefore patient is believed to have COPD (Vestbo, 2010). Although all the sign and symptom of the client signify that patient is suffering from COPD still functional diagnosis is necessary to confirm the exact diagnosis and proper physical assessment is necessary to confirm the diagnosis (Ferrara, 2011). In high risk cases of COPD proper cardinal sign, body mass index, height and weight of the patient is to be notified as component of assessment (Stockle, 2007). There are some other drugs that are beneficial in severe stage such as antioxidants and immunoregulators (Gold, 2008) Improper nutrition in COPD patient is quite general and it has harmful influence on the pulmonary functioning. So helping the client in maintenance of adequate nutrition level is important and beneficial in improving the respiratory functioning (Shepherd, 2010). Moreover the client whose BMI is less than 21 kg/m2 should be given supplementary diet in order to improve condition of client further (Vermeeren et al, 2001). However age associated alteration in structure, height, muscular changes and if value of BMI is more than also malnutrition cannot be identified (Shepherd, 2010). In patient BMI is in normal category instead patient is advised to take adequate calories, carbhohydrate and proteins in diet. Patient is also advised to avoid caffeine, restrict sodium and taking more milk. The other preventive measure is vaccination of influenza. It has much influence in prevention of various acute respiratory illnesses along with COPD. This type of vaccination must be advised to each patient suffering from COPD (Wongsurakiat, 2004) . Patient who is moderate or severe deterioration of lung function then pulmonary rehabilitation is beneficial to gain maximum functioning to manage with disease condition. Patients are motivated to do different types of exercise to keep body healty Such as steps up, walking , various arm exercises, self handling of various activites such as taking medication, that helps in keeping the function of different system appropriately. Also exercise for decreasing hyperinflation and manage dyspnoea such as deep breathing and pursed lip is advised to the patient (Barnett, 2008). Moreover advantages of rehabilitation of lung fuction maintain for numerous months even after the end of this program (O` Donell, 2007) . COPD is the most prevalent problem that effect mostly elderly people. The mai symptom of COPD is shortness of breath, production of cough with sputum. The main cause of occurrence of this disease is cigarette smoking. History taking and physical examination is beneficial in excluding all the differential diagnosis. Through spirometry diagnosis of COPD can be made. Pharmacology is helpful in controlling the further spread of disease but medication does not provide efficient result in long term care. CASE STUDY ON SPINAL CORD INJURY This case study will throw light on the complete physical examination and diagnostic test and a range of laboratory investigation to confirm the diagnosis. It will then undergo detail treatment plan along with differential diagnosis to explain the appropriate intervention on the basis of present evidence sources. Also it will explore the recent issues regarding care of spinal cord injured patent. The actual name of the patient is changed in the study so that data cannot be recognized (Dimond, 2002). Mr. Malik Aggarwal is a 21 year old student studying in University of Greenwich doing mastering in marketing. During summer break in France, Malik was struck with an motorbike accident. During accident he fall harshly on the footpath from his bike and was lying towards his abdomen . This accident was witnessed by the policeman and he quickly gave call for first aid management. Ambulance was arrived soon and emergency staff identify the severity of Malik injury, they protect the neck of the victim with the help of strong board and immobilize the part before transferring him to the emergency department of the closest hospital. The assessment was carried out by the eme rgency specialist staff, and found that there were contusions and lesions present on his trunk and lower extremities, the restriction of the neck was rapidly maintained by the staff with the help of firm cervical collar. In order to limit further deterioration of condition, patient was supported with cervical traction and head was also supported with motion restricted devices that are helpful in maintaining proper alignment (Grubb, 2006) . The cardinal signs were checked and indicate that patient has Blood pressure of 100/70, pulse rate: 90/minute, respiration: 20/mt, temperature: 98.4 F and oxygen saturation was 90%. Proper pharmacological history , family history and dietary history should be taken because it might be beneficial in knowing the disease (Cox, 2008) . History collection of patient reveals that patient was anxious and had complaint of pain at the site of neck. The constant pain occur due to the contraction of muscles of neck muscls. There were presence of mild lesions on the trunk and extremities. Therefore it might be the case of injury on cervical cord because continual pain is the indication of spinal cord injury (Lee and Ostrander, 2003) . The exact cause of injury is the extreme flexion of head towards the chest that is due to hyperflexion. Moreover if the main reason of spinal cord injury is hyperflexion or hyperextension then it might be the case of spinal cord injury (ElFaramawy, 2009). The client symptoms of hypotension, bradycardia indicates that it might be due to neurogenic shock (McLeod, 2004). After the collection of data it is essential to carry out physical assessment by covering all the aspects. Adequate Physical assessment is the basis in determining the injury related to spinal cord (Bono and Lee, 2004). Moreover physical examination especially of neurological system is beneficial in gathering the baseline data. After that cranial nerves should be assessed by various method such as recognization of smell, assessment of the vision through snellen chart, assess for speech ,facial expression, assess for optic fundi with the help of opthalmoscope, if there is presence of papilloedema it may be the case of increase intracranial pressure. In patient all the cranial nerves are intact and also no papilloedema is present which exclude the diagnosis of increased intracranial pressure (Cox, 2008) . Patient motor and sensory function is determined by the American Spinal Injury Association (AISA) impairment scale. In this scale sensory deficit and motor impairment is recognized with the help of broad categorization. In the patient the strength of elbows, wrist was normal, the stretch of the biceps and triceps was also normal. The CT scan is beneficial in visualization of the image in different cross section (Jorge, 2009). It is possible to get exact information regarding injury of bone with the help of CT scan and radiography films however the injury to the soft tissue on the cord, intervertebral disc then these diagnostic test does not provide complete information regarding soft tissues (Sheerin, 2005). The CT scan of patient exhibit that injury take place at the level of C5 spine. Magnetic Resonance Imaging is helpful in visualization of injury that occurs on soft tissues by providing well clarify image. The main aetiology of spinal cord injury is straight mechanical injury and injury develops due to the compression on cord (Pellatt, 2010). This compression develops from traumatic and non traumatic reason (McDonald Sandusky, 2002). Due to this overstress of cord, intense flexion, extension or rotation develops. This will further causes development of oedema (Bailes et al, 200) . Management of the patient begins from the site of the accident. First priority of the management is the maintenance of the airway, breathing and circulation. Oxygen therapy helps to prevent hypoxic conditions along with that oxygen therapy stop secondary injury that occurs due to the hypoxic conditions. For maintain airway jaw thrust method should be used chin lift method should be avoided as this may deteriorate the neck by extension (Pellatt, 2010). Immediately stabilization of spine must be maintain with the help of stiff cervical colour. The client must be positioned on hard board so as to maintain proper alignment. If there is requirement of intubation then it should be maintained with the help of three people without mobilizing the patient. However immobilization is not as important as the oxygen therapy because death of the patient may take place due to development of encephalopathy in hypoxic condition (Sheerin, 2005). After the confirmation of diagnosis, drug therapy should be started without any delay. Currently this drug therapy is recognized as basic treatment and helps in limiting secondary damage due to hypoxic condition. Steroid treatment must be started immediately.
